Thank you for your interest in receiving biblical counseling. Our weekly counseling appointments are offered in-person in our offices located at 9230 Ridge Rd. New Port Richey, FL 34654 in the Eduducation Building. In order to prepare for your first visit, we need you to complete the following Counseling Intake Form.

This form will take you approximately 20-45 minutes to complete. Please take your time to fill this out with as much detail as possible. Our counselors will safely guard this information and use it to help you. Filling out the form is one of the most important steps you can take towards receiving the help you need.
